Custom Forms

The Custom Forms Module within PAETEP allows you to build a variety of district specific forms for teachers, NTPEs, and supervisors. 


For example, the Custom Form Module can be used to build district specific walkthrough or 'look for' forms.  You can build forms to survey staff and monitor the progress of district initiatives.  In addition, you can build forms to facilitate professional growth plans or individual improvement plans. 



The Custom Form Module can be combined with the Support Staff Module to add and expand your evaluation capabilities to include paraprofessionals, administrative staff, personal care aides, food service employees, custodial staff, maintenance staff, clerical staff, school safety personnel, technicians and more.  


The Custom Form Module includes Templates.  A variety of templates exist, including general templates and templates for evaluating various support staff personnel. For a complete list of templates, see the FAQ section below. 


The data collected from completed custom forms can be viewed via Usage Reports. These reports can be download for further review and analysis.

Step-by-Step Guide

Custom Forms are built with two workflows:

 

Supervisor Form

Supervisor Forms are started and completed by Supervisors. This workflow can be used for evaluations, walkthroughs, and more!


Step 1 - Supervisor initiates Supervisor Form

Step 2 - Supervisor fills out form, authenticates... and Submits

Step 3 - Employee reviews, adds comments, and authenticates



Individual Form

Individual Forms are started and completed by Employees. This workflow can be used for Surveys, Self-Ratings, Feedback Forms, Reflections, Improvement Plans, and more!

Step 2 - Employee fills out form, authenticates... and Submits

Step 3 - Supervisor reviews, adds comments, and authenticates



Important Note - All Custom Forms are one way forms.  That is, either the supervisor (Supervisor Form) or the employee (Individual Form) completes the form and submits to the other party for review.  The other party is not allowed to complete a portion or section of the form.  However, all Custom Forms have a General Comments section at the bottom of the form that can be enabled so that either party can share comments back and forth.

Q: How can I use Custom Forms to evaluate support staff in PAETEP?

Custom Forms are the perfect companion to the Support Staff Module for PAETEP. With the purchase of this module, you will gain the ability to onboard your non-certified staff into PAETEP, including paraprofessionals, administrative staff, personal care aides, food service employees, custodial staff, maintenance staff, clerical staff, school safety personnel, technicians, and more!


Once these users are onboarded, your district can use the Custom Form module to create evaluation tools. If you do not currently have an evaluation, you can use one of our template forms or customize it to fit your needs.

Q: What types of questions can be used in a Custom Form?

Currently, the Custom Form Builder supports the following question types:

  • Contextual Information (autofills for employee name, supervisor name, building, completed date)

  • Electronic Signatures
  • Multiline Open Ended Response

Q: Why can't I view a Custom Form?

All Custom Forms have two stages: Draft and Completed. While a form is in the Draft stage, only the user "Waiting on..." user will have viewing permissions. Once that user submits the form, all other users will gain the ability to view the form, add comments to the General Comments and Updates section, and authenticate.


If you are unable to open/view a custom form, ensure that the "Waiting on..." user has submitted the form.

Q: Why can't I see data in the Usage Report?

Custom Form Usage Reports are a convenient way to track your form usage in one centralized location. When viewing a Usage Report, users with District Admin level privileges will see data from all instances of the form. Other supervisor, such as Principals and NTPE Supervisors, will only have access to data associated with their building assignment. 


If you are a Principal/Supervisor who needs to view Usage Report data associated with another supervisor, please contact your District Admin/Portal Manager.

Q: How can I view a Custom Form when completing a 13 form?

A Custom Form will be visible in the quick view panel of a 13 form under the "Custom Forms" header. See image below for details.




Note that only completed observations will populate this panel. If you are not seeing your data in the quick view pane, please ensure that your Custom Form is submitted/closed.
