Anecdotal Notes provide a means for supervisors and/or teachers to document and share additional evidence that might not be observed during a Walkthrough or Formal Observation. Anecdotal Notes are initiated and completed as desired by supervisors and/or teachers.
For example, teachers can use Anecdotal Notes to share evidence about their practice, including authentic assessments, unit plans, communications with families, or anything important to note, allowing them to build a portfolio of artifacts. Similarly, supervisors can use Anecdotal Notes to document positive aspects about a teacher’s practice or leadership within the school/district.

Step-by-Step Guide
Step 1 - Supervisor and/or Employee (Teacher/NTP) creates an Anecdotal Note, enters comments and uploads evidence, and submits the completed Anecdotal Note to the receiving user.
Step 2 - Receiving user reviews the completed Anecdotal Note and enters follow-up comments/evidence if appropriate, and submits.
Step 3 - Supervisor and/or Employee returns to the completed Anecdotal Note in the future, enters additional comments/evidence, and resubmits to the receiving user... or... creates a new Anecdotal Note.

Q: How can I view an Anecdotal Note's evidence when completing a 13 Form?
Evidence from Anecdotal Notes are easily accessible on 13 rating forms via the Observation Summary evidence panel on the right hand side of the form. Simply click on the Anecdotal Notes section and view completed Anecdotal Notes as needed. See image below for details.

Q: Can an Employee see what I wrote on an Anecdotal Note?
Yes. Anecdotal Notes will be viewable on the Employee's user summary page after it has been submitted.
Note: If an Anecdotal Note has a status of "In Draft", only the person who is drafting the note will be able to view it.
